The labs allowed by the ICMR are Thyrocare Mumbai, Suburban Diagnostics Mumbai, Metropolis Healthcare Ltd Mumbai and Sir HN Reliance Foundation Hospital and Research Centre, Mumbai.

The list was issued a day after the government said it would allow private labs to undertake tests for Covid-19.

The government has also said tests should be priced under Rs 4,500, with a maximum of Rs 1,500 for an initial screening test and Rs 3,000 for a confirmation test.

In a move that allows NABL-accredited private laboratories to start testing COVID-19 samples, the government issued guidelines, capping the cost of sample testing by private labs at Rs 4,500.

The guidelines issued by the Indian Council of Medical Research, affiliated with the Ministry of health, lays down the rules for sample collection, testing, reporting and sample storage and destruction.

The National task Force has recommended the maximum cost for COVID-19 samples testing at private labs at Rs 4,500. “This may include Rs 1500 for a screening test for suspect cases and an additional Rs 3,000 as confirmation test charges,” the notification said.

While capping the price, ICMR also encouraged private labs to conduct ‘free or subsidized testing in this hour of national public health emergency’.

Sources said more than 50 private laboratories have submitted their requests to the government to allow them to start tests for COVID-19.

The government is yet to release the list of authorised private laboratories.

“Preferably home collection of samples may be done by private labs to avoid suspect cases to be coming in contact with other people,” the notification said.

The guidelines recommended only real-time PCR-based assays for COVID-19 testing. They added conventional PCR, in-house real-time PCR and antibody/antigen tests are not recommended.

The real-time polymerase chain reaction (RT-PCR) test recommended by ICMR is performed on  swab samples from the inside of the throat or nose, a nasal or tracheal aspirate solution, sputum or blood samples.

The time taken to test samples has come down to 4.5 hours with the real-time PCRs. Overall, it may take over 24 hours or less from time the samples are collected to when the report is delivered.

With rapid kits now being approved, like the one by Roche Diagnostics, the time to conduct and report test results could come down to under 5 hours, thus allowing faster turnaround and larger number of samples tested.

The authorised private laboratories will also be sending all COVID-19 positive samples to NIV, Pune, under suitable biosafety and biosecurity precautions laid down by ICMR.

Negative samples will be destroyed within a week of collection.

A total of 111 government labs across the country are currently carrying out COVID-19 testing, Joint Secretary of Health Ministry, Lav Agarwal said on March 21.

India has reported 283 confirmed COVID-19 cases, including 4 fatalities and 22 recoveries.

Out of these 12 new patients, eight were from Mumbai, two from Pune and one each from Kalyan (a town near Mumbai) and Yavatmal, he said.

The Indian Council of Medical Research (ICMR) has issued directions that “community spread” of the virus be checked, the minister informed.

Six out of eight new patients from Mumbai had overseas travel history, one is airport staff while another had traveled to Gujarat, Tope said. The patient from Yavatmal, who is admitted in a Mumbai hospital, had recently traveled to Congo in Africa.

The patient in Kalyan is the brother of a woman from neighbouring Ulhasnagar who tested positive for the virus infection two days ago. Both had returned from Dubai.

In Pune, a 25-year-old man who had traveled to Ireland and England tested positive. The other person in Pune who tested positive on Saturday was a 41-year-old woman who has no overseas travel history, the minister said.

As per the latest figures, he said, Pimpri Chinchwad near Pune has 12 coronavirus patients, while in Pune city there are 11 patients. In Mumbai, the number of patients is now 19, highest in the state. Nagpur, Yavatmal and Kalyan have four Covid-19 patients each.

Navi Mumbai has reported three cases, Ahmednagar two, and Panvel, Thane, Ulhasnagar, Aurangabad and Ratnagiri one each. One coronavirus patient died in Mumbai last week.

Earlier in the day, when the number reached 63, the minister had said that rise from 52 to 63 within one one day was “big”. Among the patients detected in Maharashtra so far, over a dozen were ones who had come in contact with infected persons while others had foreign travel history, he said.

“The spread has been largely because of people who came from outside. I appeal to the people not to venture out. They should exercise self-discipline by maintaining social distancing and hygiene,” he said.

“If there is no reduction in crowding in public transport, a shut-down will have to be put in place. There is also the option of allowing people in public transport after checking their I-cards,” he said.

The suburban trains in Mumbai will function for essential activities, he added.
“If people don’t listen and continue to use public transport unnecessarily, we may have to think otherwise,” he said, adding that we are in Phase-2 and heading towards Phase-3 of virus epidemic when number of patients spurts.

The World Health Organisation and the Centre’s guidelines say that the virus survives longer in cooler places, he said. “So, not just government offices, but people (at home and offices) should also follow the advice not to use air
conditioning systems,” he said.

He also said that the rush of migrant labourers at railway stations to head back to their home states is something to worry about. “We have asked for increasing the number of outstation trains,” he added.

As to crowding in local trains, he said, “We are in observation and appeal mode as of now. Even the Centre feels that the public transport in Mumbai needs to be shut down. This was conveyed to (NCP chief Sharad) Pawar by Union Health Minister Harsh Vardhan,” he said.

Maharashtra Chief Minister Uddhav Thackeray and Pawar were in touch with the Centre, Tope added. “Private labs should be given permission to conduct
testing and even medical college hospitals can do it. This will reduce the waiting period for reports,” he said.

“Planned surgeries in civic-run and medical college hospitals have been put off. As many as 7,000 quarantine beds have been added in the state,” he said.

Government, last week, had started discussions with private labs, in an attempt to expand testing facilities. However, while ICMR had advised all private labs to conduct free of cost testing, only some of them agreed on the proposal.

According to the government, availability of testing equipment, mainly probes (as they are imported), is a criteria for any NABL-accredited private lab to come on board and start testing coronavirus.

Till last week, ICMR had utilised its own funds, to the tune of over Rs 20 crore, to conduct tests for coronavirus and had not received the sanctioned amount of Rs 20 crore from the government till last week, according to the officials.

As of 5 pm of March 20, there are 206 active cases of coronavirus have been reported in the country and social distancing has been considered the biggest and most important directive to prevent the disease transmission.

More than 1 lakh people are currently under community surveillance and nearly 6,700 people are also under surveillance as they were in contact with positive cases of coronavirus in the country.

In light of frequent cases of citizens flouting norms, states are authorised to impose a penalty of Rs 1,000 or imprisonment of six months or both on the concerned citizens, according to senior government officials.

There is no evidence of community transmission of coronavirus in India, officials said, adding that the travel history of a positive coronavirus patient in Tamil Nadu is yet to be ascertained and the Source of the virus is not yet clear in that case.

“Till date, a total of 826 samples of the people suffering from severe acute respiratory infection (SARI)/influenza like illnesses have been tested at the sites. None of the samples were found positive,” ICMR said.

Currently, ICMR is conducting tests for community transmission of COVID-19 at 51 surveillance sites. It will expand it to more regions where coronavirus cases have been reported.

ICMR, the apex health research body of India, has been the nodal authority for testing patients for COVID-19.

The council said that it has been closely monitoring the presence of community transmissions in the country and initiated sentinel surveillance to detect community transmission of COVID-19 beginning February 15.

“The surveillance sites have been scaled up from 16 sites till February 29, 2020 to 51 sites by March 15, 2020,” ICMR added.

By Thursday afternoon India had reported 180 positive cases of coronavirus, including three casualties.

However, India’s premier research organisation Indian Council of Medical Research says, “WHO’s advise of more testing does not apply to countries like India.”

At a time when India is seeing its ‘reported’ positive cases grow at a fast pace, this statement sounds a bit bizarre. While 139 positive cases (till 7:00 pm on March 16 looks a small fraction, it is growing at a fast pace. The number of positive coronavirus cases has gone up by 26 percent in just two days.

The government cannot be pinned down on its tackling this health crisis. It has been proactive in closing our borders to affected countries, closing down non-essential public place, schools, advising home quarantines, social distancing, work from home but it has been slow in testing.

With 11,500 tests in the last two months, for a country with 1.3 billion population, India stands too far behind in testing for COVID-19. “India is not in a community transmission stage so such statements are not valid for countries like ours. We can’t say that community transmission will not happen.” Experts say we will never know this until we test more people.

A data compiled by Our World in Data suggests India is only testing three per million people. This was based on March 6 figures when India had tested roughly 5,000 samples. The numbers have more than doubled in the last 10 days but is still abysmally lower.

There has been enough global experience to show that all the countries that have been able to mass test people have been better prepared and have been able to substantially slow the spread of the virus.

Case in point South Korea. It has tested over 2.5 lakh people in the last 2.5 month and used extensive surveillance and contact tracing, mass free testing and drive through testing centres to hasten the process. Compare it to those nations who didn’t do mass testing and detect fewer cases and therefore have apparently higher case mortality rates. Like Italy, like the US now.

For India, it is the 2 Cs that are troubling cat this point – COVID-19, Capacity of Testing and Criteria.

Till now the discussions hovered around testing capacity. However, in the last two days, ICMR has explained that we have enough and more testing centres. Currently, 72 government labs are testing COVID-19 samples. The government is adding 49 other government labs and two high rapid throughput systems. ICMR said it is also in talks with accredited private laboratories to start conducting COVID-19 tests, albeit advising them to do it for free.

At the same time, the government says its own government laboratories are not operating at full utilisation. 121 labs, for now, are running at 10-12 percent utilisation. Professor Balram Bhargava, DG, ICMR said the government has a capacity to test 8,000 samples per day. One would wonder why the need to add more private laboratories? And why are we not utilising the capacities?

Trouble is with the second C – Criteria. India’s COVD-19 testing strategy till now is that of risk-based assessment. This criterion of testing for COVID-19 is based on the protocol that only those travellers who have come from affected countries recently and those who have contact history with such travellers and have symptoms of the disease will be tested.

Now that is narrowing down the sample size drastically.

There have been multiple reports of travellers from various countries who wished to be tested but were sent away by the government laboratories as they don’t fit the criteria.

ICMR says we are still in the local transmission stage and there is no need for indiscriminate testing of people. There is no need for community screening. ICMR is basing its assessment on the fact that all the reported, positive cases have been through travellers or their contacts.

There are two inherent flaws with this. One: in the WHO’s daily coronavirus situation report, even Italy with 27,980 positive cases and 2,158 deaths is in the list of Local Transmission. According to WHO, local transmission indicates locations where the source of infection is within the reporting location, but not all locations within a given country/territory/area are equally affected.

Two: In the last 10 days, most of the patients who have been tested positive have had a recent travel history to Dubai (UAE). It was not on the affected countries list and hence no compulsory quarantining or testing. Just yesterday, the government expanded compulsory 14-day quarantine for people coming or transiting through UAE, Qatar, Oman and Kuwait.

As Gagandeep Kang of Translational Health Science Technical Institute puts it, “Closing borders helps in checking the spread of transmission, but will not completely control the spread of infection. Airports are points where people from multiple countries and cities transit through and there is an inherent risk or transmission. Hence we need to expand the pool and test all international travellers entering India – irrespective of the origin of travel or symptoms.”

Experts say relying on risk-based assessment is never a 100 percent and point out that we may have already entered community transmission stage considering asymptomatic patients moving around without testing, the density of population and difficulties in practising social distancing despite guidelines.

The government has started community screening in its own small way. It has now using the influenza surveillance network to test all people with influenza-like illnesses and severe acute respiratory infections. So, 51 laboratories will collect 20 samples each per week, making it about 1,000 samples per week. It’s a welcome start but a small pool.

ICMR says currently 69,500 people are under surveillance, either in quarantine centres or home isolation.

Dr Srinath Reddy of Public Health Foundation of India said, “It might be time now to move to clinical criteria, trace symptoms. China started looking at moving beyond just diagnostic-based screening to also looking at clinical criteria.”

Could the low level of testing be to delay the burden on already understaffed health infrastructure, experts say the majority of those who test positive will show milder symptoms, but we will be able to slow down the transmission with the information.

“Community transmission could’ve happened by now, we wouldn’t be able to put a number to it until we test more people,” adds Dr Kang. “We are not looking at eradicating the disease but to flatten the curve – so that more people we test, better prepared we are.”

He also appealed to all private laboratories to offer COVID-19 diagnosis free of cost.

“We have 72 functional ICMR laboratories in government sector for testing and 49 more will be active by the month end,” he said.

Bhargava said that India was in stage 2 or local transmission of the virus.
“All asymptomatic patients who have taken international flights in the past 14 days should get tested as per current protocol if they develop symptoms,” he said.

He also said that symptomatic people who come in close contact with laboratory-confirmed coronavirus cases must be tested, adding and all healthcare workers managing respiratory condition of patients should be tested too if symptomatic.

“Private NABL accredited laboratories will soon be operationalised,” Bhargava said.

ICMR was approached by several private players who want to contribute and talks are on with 51 National Accreditation Board for Testing and Calibration Laboratories (NABL) accredited private laboratories.

It has started collecting samples from its current network of 52 labs and the results of the first weekly test will be known soon.

The number of labs will be increased to 67 and soon there will be provisions for all to do not just primary tests but also confirmatory tests for coronavirus.
More than 10,000 tests have been conducted so far in the country by ICMR.

Till Friday, it had conducted nearly 6,500 tests to detect the deadly virus.
So far, India has reported 114 positive cases, as of 4 pm of March 16. Till 11:30 pm of March 15, this figure was 110. In Ladakh and Odisha reported one new case each.

Thirteen of the 114 people have recovered, according to the official data.
The Group of Ministers on coronavirus or Covid-19 also held a meeting on Monday. The government has advised states and union territories to take measures such as avoiding mass, religious gatherings, close schools, and gyms.

Restaurants have been advised to keep a distance of 1 metre between tables and companies have been asked to allow employees to work from home wherever possible.

Advisory has also been sent to maximize social distancing in trains, planes, and buses.

Currently, there are 51 labs in the country which can conduct primary tests for the novel coronavirus. Out of these 51, the second test or the confirmatory tests can be conducted at 31 labs.

“We are conducting 60-70 tests every day across the entire network of 51 labs as of now. But, we can scale it up. Each lab has the potential to conduct 90 tests every day,” ICMR said.

Earlier, National Institute of Virology (NIV) in Pune was the only lab which could confirm the presence of coronavirus in a person. ICMR has been able to gradually but quickly ramp up this number to 13, then 18 and now, a total of 31 labs can perform the confirmatory tests.

ICMR also said that rapid diagnostic kits are also being sent to NIV, Pune for validation and if these are approved by NIV, then these will also help in creating a widespread network of preliminary detection of the virus.

India has also become the fifth country in the world after China, Thailand, US and Japan to isolate the novel coronavirus and hence, this will pave the way for the development of its vaccine, ICMR added.

“As per our tests so far, the virus in India is 99.99 percent similar to Wuhan virus. Hence, lessons from China will be useful. China and Italy are at the stage where the outbreak has been such that it has resulted in an epidemic there. India is at stage 2 and there is a window of opportunity where we all together can contain it at Stage 2 which involves local transmission, not community transmissions,” ICMR added.

ICMR is also keen on keeping an eye on preventing community transmission and hence, it will start conducting weekly tests from samples taken across its 51 labs, from people with no travel history.

As of March 13 morning, ICMR said it has conducted 6,500 tests across 5,900 individuals and out of which, 78 tests were positive for the novel coronavirus. As of 5: 00 pm of March 13, there were 81 cases of coronavirus, out of whom 10 have recovered.

A 76-year-old man from Karnataka, who died on March 10 and had a travel history of Saudi Arabia, also tested positive for coronavirus. “Death of a 76-year-old male from Karnataka is confirmed to be caused due to co-morbidity and has also tested positive for COVID-19. The details of the case are as follows: He visited Saudi Arabia from 29th January 2020 to 29th February 2020,” the government said on March 12.

As far as testing facilities and material is concerned, ICMR said as of now, government labs are more suitable for conducting the tests for coronavirus and hence, private sector help has not been sought and enough inventory is available. While most testing material is made in India, probes are being imported from Germany but as of now, sufficient stock is present, ICMR said.

Asked on why only people who show symptoms are being tested, ICMR clarified that diagnosing asymptomatic people can give an impression of false security and hence, it is restricting itself to symptomatic cases only.

It is interesting to note here that before 2008 all samples for such deadly diseases used to be sent outside for testing, but ICMR has the capability now to conduct these tests in the country.

ICMR also reiterated that N95 masks are useful for people who are directly in proximity to someone who is suffering from coronavirus or who is a suspected patient. People should not hoard masks, ICMR added.

ICMR, the apex medical research body of India, will collect samples from 51 labs across the country every week and these samples will be from people who show signs of influenza-like illness, severe respiratory issues and have not travelled to any foreign country recently.

ICMR has already been conducted these tests from mid-February but the frequency will increase to once a week from once in 15 days earlier. The move is aimed at checking and preventing community transmission of the novel coronavirus or COVID-19 in the country.

As of now, India is in Stage 2 where there are cases of local transmission or where only those people are suspects who have either travelled to a foreign destination or who are in close proximity with someone suffering from coronavirus.

As of 5:00 PM of March 13, India had a total of 81 positive cases of COVID-19, of which 10 have recovered and rest are stable.
